The Aged Care Home Package Service includes:

Household management includes cleaning and organising your home and providing laundry services.
Meal preparation: If you need help preparing meals for yourself or others in your household, meal prep assistance
Entertainment: When you need assistance entertaining yourself or others in your household, Household Tasks can assist with games, puzzles, arts and crafts projects and more.
Home Maintenance Repairs: Help to maintain your home and garden, keeping it safe and tidy.
Personal Care: Support with daily self care tasks like showrering, getting dressed, and taking medications.
Respite Support: We provide you with tailored care so your family members and carers can rest and recharge.
Social Support and Community Engagement: Support to keep you socially active, connected, and independent.
Transport: We can help you travel to your appointments, the local shops, or other places of interest.
Allied Health and Other Therapeutic Services: Support to restore and maintain your strength, mobility and balance, so you can live safely and independently.
Care Management: A dedicated Care Partner to help you plan, coordinate and review your services to meet your goals.

Who is Eligible?

You'll be eligible for an aged care assessment if you are:

Over the age of 65
An Aboriginal or Torres Strait Islander person over the age of 50
A younger person with a disability, dementia or other care needs not met through other specialist services
